Than icon Emitir Certidão de Antecedentes Criminais

The following form will appear:

Nome = Name
Nome do Pai = your father´s name
Nome da Mãe = your mother´s name
Data de nascimento = your date of birth
Nacionalidade = nationality
Naturalidade = State you were born UF: Capital
Número do Passaporte = passport number (include the full number although the space provided seems that it will not fit all numbers)
Série: 00
Carteira de Identidade = your ID if applicable
CPF =  include it only if you have a CPF number.
