You will need the ALSO KNOW AS document issued by the nearest Embassy, Consulate or Honorary Consulate.

The Federal Police, Immigration, Local Notaries and other Authorities request this document when a person has two or three different names or changed the name for civil purposes such as marriage or divorce.

Documents required:

• original documents that officially show the differences in the names: passport, driving license, birth certificate or marriage certificate. certified copies will not be accepted.
